Annual Organization Renewal Forms

Event Forms

  1. Scheduling and Event Form – online form to reserve a room for meetings, events, etc.
  2. Catering Order/Event
    This form must be completed and submitted for all Chartwell’s catering orders.


Contracts

  1. 2012 Entertainment Contract Revised (Microsoft Word Document) – The contract is required when making an agreement for someone providing services, whether for a stipend or on a voluntary basis, for an event.

Waivers

  1. Alcohol Rider Policy - This form is required for events stating no alcohol or illegal substances will be provided at university approved events on/off campus.
  2. Waiver of Liability - This form is required for emergency purposes.


Financial Forms

Online Financial System

  1. Event Audit Form – This form is needed to track fundraiser sales.
  2. Record of Dues Roster – for recording dues collected and should be completed and maintained in the Treasurer’s Financial Record Book
  3. Cash Count Report - for documenting revenue from sales for profit.
  4. Event Audit Form - This form is needed to track fundraiser sales.
  5. Fund Raising Sales Agreement (Vendor % Sales Fundraiser) - for documenting revenue from sales for profit.
  6. Inventory and Sales Control Worksheet - for tracking inventory and sales for products at student organization events.
  7. Leadership Funds Application – The process to request Leadership Funds has recently changed. Please click here to download more information.
  8. Merchandise Sales Report - for documenting sales revenue and merchandise inventories.
  9. Participant Roster - for recording money collected for participation in and event and should be completed and maintained in the Treasurer’s Financial Record Book.

  11. Support Funds Application - application for obtaining support funds for a new non-selective organization.
