Change of Degree Objective

Students who are ready to change their majors should contact their current advisor with their request.  If the student meets the requirements to change, the new advisor will complete a Change of Degree Objective (CODO) form.  Nursing does not use these forms, please see below.  Students may need to see the prospective major’s academic advisor to complete the process.  The change will be made after the completed form is received by the Office of the Registrar, located in the Enrollment Services Center in Lawshe Hall, Room 130.

FOR ENTRY LEVEL NURSING STUDENTS ONLY:

Fill out the appropriate Request for Consideration by the Nursing Admissions Committee form.   The forms are below and also available in the Office of the Registrar in Lawshe Hall, Room 130.  This form must be returned to the Office of the Registrar before January 31st for consideration for the following fall term.  A copy of this form will be forwarded to the School of Nursing on February 1st along with any transcripts from other schools attended and any additional required documents that are on file.  Please read the prerequisite steps as listed on the forms carefully.  Requests for admission will be submitted to the Admission, Progression, and Graduation Committee in early March.  Students will be notified of the committee’s decision at the end of March.
