Private Education Loan Self-Certification
Application Form Information
Applications for private education loans are made by the student through individual lenders. These lenders are required by law to obtain a Private Education Loan Applicant Self-Certification Form (PDF) from the applicant prior to disbursing a private education loan.
Instructions:
- Obtain the Private Education Loan Applicant Self-Certification Form from your lender, or print the form from the link above.
- In Section 2 of the form provide your Cost of Attendance and Estimated Financial Assistance. *
- Complete Sections 3 and 4 of the form.
- Submit the completed form to your lender; do not submit this form to the Office of Financial Aid and Student Accounts.
* To obtain your Cost of Attendance for Section 2, Item A:
- If you filed a Free Application for Federal Student Aid (FAFSA):
- Login in to PCSTAR [Opens in New Window].
- Select “Student Services and Financial Aid” from the PCSTAR Main Menu.
- Select “Financial Aid”.
- Select “Cost of Attendance”. For a full year loan, enter the “Total” as your cost of attendance on Section 2A of the form.
- If you did not file a FAFSA or if your Cost of Attendance is not available on PCSTAR
- Refer to the Cost of Attendance table to determine your cost of attendance.
To obtain your Estimated Financial Assistance for Section 2, Item B:
- Login in to PCSTAR [Opens in New Window].
- Select “Student Services and Financial Aid” from the PCSTAR Main Menu.
- Select “Financial Aid”.
- Select “Award Information”.
- Select “Awards by Aid Year” and make a note of your Total Financial Aid Award.
- Select “Resources/Additional Information” and make a note of Total Expected Resources.
- Add the Total Financial Aid Award and the Total Expected Resources. This is your Estimated Financial Assistance for Section 2B of the form.
Complete Section 2, Item C following the instructions on the form.
