Verification (Independent)

You have been selected by the U.S. Department of Education for a process called Verification. Verification is a process to review the accuracy of the information reported on your Free Application for Federal Student Aid (FAFSA) by comparing it with your (and your spouse’s, if you are married) Federal income tax information and/or other financial documents.

Instructions:

  1. For 2014-15, print the 2014-15 Independent Verification Worksheet (pdf) .
  2. For 2013-14, print the 2013-14 Independent Verification Worksheet (pdf) .
  3. Submit the completed and signed worksheet along with any additional documentation requested to the Office of Financial Aid and Student Accounts at Purdue University Calumet- Lawshe Hall Room 130.

Note: Once all documents are received and reviewed we may need to make electronic corrections to your FAFSA information.

Remember to include your PUID and name on all documentation submitted to the Office of Financial Aid and Student Accounts at Purdue University Calumet.
