- For 2013-14, print the 2013-14 Dependent Verification Worksheet (pdf) .
- Submit the completed worksheet signed by you and a parent along with any additional documentation requested to the Office of Financial Aid and Student Accounts at Purdue University Calumet – Lawshe Hall Room 130.
