Untaxed Income Follow-up (Parent)
You have been selected by the U.S. Department of Education for a process called Verification. Verification is a process to review the accuracy of information reported on the Free Application for Federal Student Aid (FAFSA). A discrepancy exists between the amount of untaxed income your parent(s) reported on the FAFSA and the amount reported on your Verification Worksheet. Further processing of your financial aid will not occur until the information requested below has been received by the Office of Financial Aid and Student Accounts.
Instructions:
- Print the Untaxed Income Follow-up (Parent) Form (PDF).
- Submit the completed form to the Office of Financial Aid and Student Accounts.
Important: Remember to include your PUID and name on all documentation submitted to the Office of Financial Aid and Student Account at Purdue University Calumet.
