Based on your Free Application for Federal Student Aid (FAFSA), your undergraduate/graduate status was unable to be determined. Further processing of your financial aid will not occur until the information requested below has been received by the Office of Financial Aid and Student Accounts.

Instructions:

Print the Undergraduate/Graduate Status Form (PDF).
Submit the completed form to the Office of Financial Aid and Student Accounts.

Important: Remember to include your PUID and name on all documentation submitted to the Office of Financial Aid and Student Accounts at Purdue University Calumet.
