Submit your parent(s) 2011 Federal IRS Tax Return Transcript if your parent(s) are required to file federal taxes. Parent(s) may request a federal tax return transcript from the IRS on-line at www.irs.gov or by calling 1-800-908-9946.

IRS requirements concerning who must file a Federal tax return may be found in Part 1 of IRS Publication 17 (PDF) .
Important: Remember to include your PUID and name on all documentation submitted to the Office of Financial Aid and Student Accounts at Purdue University Calumet.
