Social Security Card Documentation (Mother)

Instructions:

Submit either one of the following documents to the Office of Financial Aid and Student Accounts:

A copy of your mother’s signed social security card, OR
A letter from the Social Security Administration stating your mother’s social security number and name

Important: Remember to include your PUID and name on all documentation submitted to the Office of Financial Aid and Student Accounts at Purdue University Calumet.
