Social Security Card Documentation (Father)
Instructions:
Submit either one of the following documents to the Office of Financial Aid and Student Accounts:
- A copy of your father’s signed social security card, OR
- A letter from the Social Security Administration stating your father’s social security number and name
Important: Remember to include your PUID and name on all documentation submitted to the Office of Financial Aid and Student Accounts at Purdue University Calumet.
