The copy of the federal income tax documents you provided indicates your parent(s) received a pension/annuity. Additional information is being requested. Further processing of your financial aid will not occur until the information requested below has been received by the Office of Financial Aid and Student Accounts.
- Print the Pension/Annuity Follow-up (Parent) Form (PDF).
- Submit the completed form to the Office of Financial Aid and Student Accounts
Important: Remember to include your PUID and name on all documentation submitted to the Office of Financial Aid and Student Accounts at Purdue University Calumet.
