Verification Worksheet (Independent)

You have been selected by the U.S. Department of Education for a process called Verification. Verification is a process to review the accuracy of the information reported on your Free Application for Federal Student Aid (FAFSA) by comparing it with your (and your spouse’s, if you are married) 2011 Federal income tax information and/or other financial documents.

Your application will be placed on hold until the requested documentation is provided. Please submit the requested documents to the Office of Financial Aid and Student Accounts as soon as possible, but no later than September 1, 2013. Failure to comply will result in a cancellation of registration and any aid awarded will be cancelled.

Instructions:

  1. Print the Independent Verification Worksheet (pdf) .
  2. Submit the completed and signed worksheet along with any additional documentation requested to the Office of Financial Aid and Student Accounts at Purdue University Calumet.

Note:

Once all documents are received and reviewed we may need to make electronic corrections to your FAFSA information.

Remember to include your PUID and name on all documentation submitted to the Office of Financial Aid and Student Accounts at Purdue University Calumet.
