Verification Worksheet (Dependent)

You have been selected by the U.S. Department of Education for a process called Verification. Verification is a process to review the accuracy of the information reported on your Your Free Application for Federal Student Aid (FAFSA) by comparing it with your and your parent(s)’ 2011 Federal income tax information and/or other financial documents. Based on this review Purdue University Calumet may need to make electronic corrections to your FAFSA information.

Your application will be placed on hold until the requested documentation is provided. Please submit the requested documents to the Office of Financial Aid and Student Accounts as soon as possible, but no later than September 1, 2013.  Failure to comply will result in a cancellation of registration and any aid awarded will be cancelled.

Instructions:

  1. Print the Dependent Verification Worksheet (pdf) .
  2. Submit the completed worksheet signed by you and a parent along with any additional documentation requested to the Office of Financial Aid and Student Accounts at Purdue University Calumet.

Note:

Once all documents are received and reviewed we may need to make electronic corrections to your FAFSA information.

Important:

Remember to include your PUID and name on all documentation submitted to the Office of Financial Aid and Student Accounts at Purdue University Calumet.
