Citizenship Documentation

Instructions:

Submit one of the following acceptable forms of documentation of U.S. Citizenship to the Office of Financial Aid and Student Accounts:

A copy of:

  • Your (the student’s) birth certificate showing that you were born in the United States which includes Puerto Rico, Guam, the U.S. Virgin Islands, American Samoa, Swain’s Island or the Northern Mariana Islands
  • Form FS-240 (“Consular Report of Birth Abroad”)
  • Form FS-545 (“Certificate of Birth-Foreign Service”)
  • Form DS-1350 (“Certification of Report of Birth”)

OR

The original of:

  • A United States passport, current or expired, except limited passports. (In the case of nationals who are not citizens, it will be stamped “Non-citizen National”.)
  • Form N-560 or N-561 (“Certificate of Citizenship”), which contains at least the following information:
    • Student’s name
    • Certificate number (found in the upper right hand corner)
    • Date of issuance of the certificate
  • Form N-550 or N-570 (“Certificate of Naturalization”), which contains at least the following information:
    • Student’s name
    • Certificate number (found in the upper right hand corner)
    • Alien Registration Number (A-number)
    • Name of the court where the naturalization occurred
    • Date of naturalization

Note:

  • U.S. Citizenship and Immigration Services (USCIS) has confirmed that any of the above documents may be photocopied as long as it is done for lawful purposes
  • Social Security cards and driver’s licenses are not acceptable forms of documentation of citizenship
  • The Office of Financial Aid and Student Accounts is required by Federal Regulations to review the ORIGINAL document/s related to your citizenship status. Please call (219) 989-2301 to schedule an appointment to meet with Cindy C. or Tanika in the Office of Financial Aid and Student Accounts between 8am-4pm Monday thru Friday. Thank you.

Important:  Remember to include your PUID and name on all documentation submitted to the Office of Financial Aid and Student Accounts at Purdue University Calumet.
