Child of Indiana Disabled Veteran or POW/MIA Fee Remission


The Office of Financial Aid and Student Accounts at Purdue University Calumet will request this authorization to determine if you wish to use your Child of a Disabled Indiana Veteran or POW/MIA (CDV) benefits for the upcoming academic year.

Students identified by the Office of Financial Aid and Student Accounts as prior CDV recipients will be requested to complete a CDV Authorization Form. Children of certain veterans are eligible to receive CDV benefits for a maximum of 124 credit hours. Visit the State Student Assistance Commission of Indiana (SSACI) website at www.in.gov/ssaci  on the eStudent link to check the official count of used CDV credit hours.

Instructions:

  1. Submit a Free Application for Federal Student Aid (FAFSA) for the appropriate academic year on-line at http://www.fafsa.ed.gov/  if you have not already done so. Be sure to release your FAFSA data to Purdue University Calumet (School Code 001827). A completed FAFSA record must be received by the Office of Financial Aid and Student Accounts, free of error and SSACI edits, before the end of the semester for which you are requesting benefits.
    NOTE:  To receive CDV benefits for the summer semester, the FAFSA for the upcoming year is required, free of errors and SSACI edits.
  2. Check your Satisfactory Academic Progress (SAP) eligibility. Student placed in an ineligible status cannot receive any financial aid, including CDV benefits. An ineligible status can be appealed if you had mitigating circumstances such as personal injury or illness. If you wish to appeal your ineligibility, please print, complete and submit to the Office of Financial Aid and Student Accounts a Satisfactory Academic Progress Appeal form (PDF).
  3. Print and complete the CDV Authorization form (PDF).
  4. Return the completed CDV Authorization form to the Office of Financial Aid and Student Accounts.  

Processing of CDV benefits will occur unless a CDV Authorization Form has been received notifying us otherwise.

NOTE: Notify the Office of Financial Aid and Student Accounts if you make any changes to your schedule during any semester in order for the correct amount of CDV benefits to be credited to your account.

Important: Remember to include your PUID and name on all documentation submitted to the Office of Financial Aid and Student Accounts at Purdue University Calumet.
