Application Process for Students and Faculty/Staff

  1. Complete Volunteer Application Form. Submit online [Opens in New Window] or print the PDF version [Opens in New Window] and fill out by hand.
  2. Read through the Volunteer Handbook [Opens in New Window].
  3. Sign last page of Handbook (page 5). Click here [Opens in New Window] to submit an electronic signature, or submit last page to CLO 176.

 

Application Process for Community Members:

NOTE: Community members must go through background check prior to becoming a volunteer (step 4 below).

  1. Complete Volunteer Application Form. You can apply online [Opens in New Window] or print the PDF version [Opens in New Window] and fill out by hand.
  2. Read through the Volunteer Handbook [Opens in New Window].
  3. Sign last page of Handbook (page 5). Click here [Opens in New Window] to submit an electronic signature, or submit last page to CLO 176
  4. Complete the PUC Volunteer Agreement and Release Form [Opens in New Window] and the PUC Volunteer Registration Form [Opens in New Window]. Return BOTH of these forms to CLO 176 or scan/email back to Kristen.Bunner@purduecal.edu. Copies are also available in our office.
