Go online and enroll by December 31, 2010 via Fidelity’s website or contact a Fidelity Workplace Planning and Guidance Consultant at 1-800-328-6608 to assist with your retirement enrollment.

Investment decisions you have to make for your January 1, 2011 contributions.

  • Employer Contributions (10%): 403(b) Defined Contribution Retirement Savings Plan
  • Employee Mandatory Contributions (4%): 401(a) Mandatory Retirement Savings Plan
  • Voluntary Contributions (Tax Deferred Annuities): 403(b) Voluntary Retirement Savings Plan and 457(b) Voluntary Retirement Savings Plan
  • Designate beneficiaries
