Below are fees for the most common labs and services. It is in no way a complete list and fees may change if costs to the SHSC increase/decrease. If you have further questions about specific fees, please call (219) 989-1235 during normal business hours.
Office Visits
- Physical exam – $20

- Follow-up visits (per provider’s request) – NO CHARGE
